Cocoa flavanol supplements cut cardiovascular deaths — but missed their main goal

What it is. COSMOS was a large randomized trial testing whether a concentrated cocoa-flavanol extract could reduce cardiovascular events in older adults. It included 21,442 US participants and followed them for a median of 3.6 years. The product was not chocolate; it was a standardized supplement designed to deliver cocoa flavanols without sugar, fat or calories from confectionery.

What it found. The primary outcome - total cardiovascular events - was not significantly reduced. The direction was favorable, about a 10% lower risk, but it did not clear the statistical bar the trial set in advance. The more interesting signal came from a secondary outcome: cardiovascular death was 27% lower, and in participants who took the pills consistently, the reduction was reported as 39%.

Why researchers care. Cocoa flavanols can improve endothelial function, which is the ability of blood vessels to relax and respond normally. They may also have small effects on blood pressure and inflammation. That gives the finding biological plausibility, but plausibility is not the same as proof that a supplement prevents heart attacks or strokes.

How strong is it? Stronger than a food-frequency association, because COSMOS was randomized and large. But the headline benefit was secondary, not the main endpoint. Secondary findings are useful for generating confidence and guiding future trials, but they are easier to overinterpret. The clean reading is: cocoa flavanols look promising for vascular health, but the cardiovascular-outcome case is not settled.

Practical take. Unsweetened cocoa powder and very dark chocolate can be part of a healthy pattern, especially when they replace sweeter desserts. But a sugary chocolate bar is not a heart-health intervention. If a future benefit exists, it is likely tied to flavanol dose and consistency, not to candy.

What to watch next. The useful follow-up would be another long trial that treats cardiovascular death as a primary outcome, uses a clearly standardized flavanol dose, and reports baseline diet and adherence transparently. Until then, the safest interpretation is food-first: cocoa can be a bitter, polyphenol-rich ingredient in a healthy diet, but the supplement case still needs confirmation.

Bottom line. Cocoa flavanols have an intriguing heart-health signal, especially for cardiovascular death, but the main trial result was neutral. Interesting science, not a reason to eat more sugary chocolate or start a supplement automatically.
