A recent randomized controlled trial has revealed promising results for older women looking to maintain muscle health. Conducted over 12 weeks with 96 healthy women aged 65 and older — all screened to be free of insulin resistance — the study examined the effects of combining resistance exercise (RE) with essential amino acid (EAA) supplementation. This research is particularly relevant given the increasing concerns around sarcopenia and declining physical function in aging populations.
Participants were divided into four groups: a control group, a resistance exercise group, an EAA group, and a combined RE + EAA group. The intervention involved circuit-based training sessions three times a week, lasting 60 minutes each at moderate intensity. Those in the EAA and RE + EAA groups consumed 5.5 grams of EAA twice daily — 11 grams a day in total.
The combination outperformed either piece alone. Muscle mass rose significantly in the RE + EAA group (F(3, 72) = 5.042, p < 0.001, partial η² = 0.174 — a moderate-to-large effect by conventional thresholds), and senior fitness test scores improved across a range of p-values from below 0.05 to below 0.001. Myostatin, a protein that puts a brake on muscle growth, fell (p < 0.05), while follistatin, which releases that brake, rose in both the RE group (p < 0.05) and the RE + EAA group (p < 0.001). The ratio between the two — the study's headline measure — climbed most in the combined group (F(3, 72) = 5.556, p = 0.002, partial η² = 0.188), significantly ahead of control (p < 0.001) and of EAA alone (p < 0.05).
Inflammatory markers also improved. IL-6 and IL-1β decreased in both the RE and RE + EAA groups (p < 0.05), while TNF-α dropped only in the RE + EAA group (p < 0.05). These findings suggest that combining resistance training with EAA supplementation may not only enhance muscle mass but also reduce inflammation, a critical factor in aging.
The limits are worth naming precisely. This is a randomized controlled trial, so the design does support a causal reading — the caveats lie elsewhere. Ninety-six women were randomized, but the degrees of freedom reported for the main analyses point to roughly seventy-six being analyzed, meaning the per-group numbers are smaller than the headline figure suggests. The trial was registered retrospectively (KCT 0010756, July 2025), which weakens the guarantee that the reported outcomes were the ones planned from the start. The participants were healthy, metabolically intact women in a supervised program, which is the group most likely to respond; twelve weeks says nothing about whether the gains hold. And the follistatin/myostatin ratio is a blood marker, not a health outcome — it tracks the biology of muscle growth rather than proving fewer falls or fractures later.
For older women, these results highlight the potential benefits of integrating resistance training with nutritional strategies to combat age-related muscle loss. This approach may offer a proactive way to promote healthier aging and improve overall fitness.
Bottom line: Combining resistance exercise with essential amino acids appears to enhance muscle mass and fitness while reducing inflammation in older women. This may serve as a foundational strategy for preventing sarcopenia and encouraging active aging.
