The ketogenic diet's effect on blood fats isn't one story — it's two, pulling in opposite directions, and two independent meta-analyses now agree on both. Researchers pooled 53 randomized controlled trials on adults following a ketogenic diet (defined as over 45% of calories from fat, under 10% from carbohydrates) and compared it against control diets. A second, separately conducted meta-regression of 62 studies reached almost identical numbers — a rare case of two research teams independently confirming each other.
The good news first. The 53-trial analysis found keto reduced triglycerides (WMD: -22.31 mg/dL, 95% CI: -28.90 to -15.72, p < 0.001) and raised HDL-C — "good" cholesterol — by 3.52 mg/dL (95% CI: 1.24 to 5.81, p < 0.001). The 62-study analysis found nearly the same: triglycerides down 19.96 mg/dL, HDL-C up 3.61 mg/dL, and the triglyceride-to-HDL ratio (WMD: -0.31, a marker some researchers consider a better cardiovascular risk signal than either number alone) also improved.
The less good news. The 53-trial analysis found keto significantly raised LDL-C — "bad" cholesterol — by 8.22 mg/dL, and total cholesterol by 8.06 mg/dL (both p < 0.001). The 62-study analysis landed almost on the same numbers independently: LDL-C up 8.49 mg/dL, total cholesterol up 8.14 mg/dL.
The evidence isn't equally solid on both sides of that trade-off. Using the GRADE framework, the 53-trial analysis rated the triglyceride and HDL-C findings as moderate certainty, but the LDL-C and total cholesterol findings as only low certainty — worth knowing before treating the LDL rise as settled fact. One consistent pattern across both papers: the longer someone stayed on the diet, the more HDL-C tended to rise (a significant relationship in both, coefficient -0.125, p = 0.011 in the 53-trial version) — the changes aren't a brief adjustment period that fades with time.
What neither study can tell you. Both explicitly flag the same gap: there's no long-term outcome data connecting these lipid shifts to actual heart attacks, strokes, or cardiovascular deaths. A higher LDL-C is a well-established risk marker, but whether keto-induced LDL increases carry the same cardiovascular risk as other causes of high LDL hasn't been directly tested. Neither review reported a funding conflict of interest.
Bottom line. Two independently conducted meta-analyses of ketogenic diet trials agree: it meaningfully improves triglycerides and HDL-C, but also raises LDL-C and total cholesterol by a similar margin (roughly 8 mg/dL) — a genuine trade-off, not a clear win. Anyone on a ketogenic diet long-term, especially with existing cardiovascular risk factors, has a reasonable case for getting a lipid panel checked rather than assuming the diet is universally heart-healthy.
