Ketone monoester and caffeine: a boost for cognitive performance in heat stress

Recent research published in the Journal of the International Society of Sports Nutrition examined the effects of ketone monoester ingestion combined with caffeine on cognitive and physical performance during exercise in heat. This randomized, double-blind study involved seventeen endurance-trained males who underwent a series of tests to assess their performance under extreme conditions, specifically at 34 °C with 45% relative humidity.

Key findings revealed that participants who ingested ketone monoester showed improved cognitive performance in working memory tasks, particularly in the 1-back test, compared to those who consumed a carbohydrate control. The ketone group demonstrated increased accuracy and target discrimination, indicating enhanced cognitive function during heat stress. However, no significant differences were observed in more complex tasks, such as the 2-back test, nor in reaction time or object hit and avoid outcomes.

Physical performance also benefited from ketone monoester ingestion, as evidenced by a longer time-to-exhaustion during high-intensity exercise compared to the carbohydrate group. This suggests that ketone monoester may provide a dual advantage, enhancing both mental and physical endurance under challenging conditions. Importantly, while cognitive and endurance improvements were noted, cardiovascular measures like heart rate and core body temperature remained consistent across both conditions.

This research is particularly relevant for special operation forces and athletes who often train or operate in extreme environments. The ability to maintain cognitive function while enduring physical stress is crucial for performance and safety.

Practical takeaways from this study suggest that combining ketone monoester with caffeine could be beneficial for those engaged in prolonged, strenuous activities in heat. However, the results do not imply that these substances will universally enhance all aspects of cognitive performance, as some tasks did not show significant improvement.

Bottom line: Ketone monoester and caffeine co-ingestion may support specific cognitive and physical performance aspects during heat stress without affecting cardiovascular strain. Further research is needed to explore the full range of cognitive tasks and long-term implications.
