Mediterranean diet linked to lower cardiovascular disease risk than low-fat diet

Twenty years is long enough to see which diet actually holds up. Researchers followed 12,197 U.S. adults aged 55 to 80, all with diabetes or at least 3 major cardiovascular risk factors, using data from the Nurses' Health Study and the Health Professionals Follow-up Study. Diet was reassessed every 4 years with validated questionnaires. Using a technique called target trial emulation — designed to approximate what a randomized trial would show using detailed real-world cohort data — researchers estimated 20-year cardiovascular disease (CVD) risk under three sustained dietary patterns: a low-fat diet, a Mediterranean diet, and the American Heart Association's (AHA) 2020 dietary goals.

Over the 20 years, 3469 CVD cases occurred (1451 men and 2018 women) — heart attacks, coronary procedures, strokes, and CVD deaths, all confirmed by medical records. The estimated 20-year risk varied clearly by diet: 35.9% under the low-fat diet, 28.2% under the Mediterranean diet, and 31.2% under the AHA-2020 goals. Compared with the low-fat diet, that works out to a risk ratio of 0.79 for the Mediterranean diet and 0.87 for the AHA-2020 goals — meaningfully lower risk for both alternatives.

The effect was smaller, but still real, in people without elevated risk to begin with. In a general population not selected for age, diabetes, or existing CVD risk factors, the same comparison produced risk ratios of 0.90 for the Mediterranean diet and 0.94 for the AHA-2020 goals versus low-fat — a more modest gap, but the direction held.

This is not a randomized trial, and that distinction matters. Target trial emulation is a more rigorous method than a simple observational comparison — it's built specifically to reduce the biases that plague diet-and-disease studies — but it still relies on what people reported eating, not on being randomly assigned to a diet and followed under controlled conditions. People who sustain a Mediterranean-style diet for 20 years likely differ from those who don't in other ways the analysis can't fully capture, even with statistical adjustment.

Bottom line. Among older U.S. adults at elevated cardiovascular risk, an estimated 20-year risk of 28.2% under a sustained Mediterranean diet compares favorably with 35.9% under a low-fat diet — a risk ratio of 0.79. The benefit was smaller but still present in a general, non-risk-selected population. This adds to a substantial existing body of Mediterranean diet research, though as an emulated rather than a randomized trial, it's strong supporting evidence rather than definitive proof.
