Whole dietary pulses may help lower cholesterol levels

A recent systematic review and meta-analysis published in the Journal of the American Heart Association examined the effects of whole dietary pulses on cholesterol levels. It pooled 38 trials (52 trial comparisons, n=2095) running at least three weeks, with a median duration of 6 weeks. The dose participants actually ate matters here: a median of 130 g of pulses per day — a modest side portion of beans, lentils, or chickpeas rather than a wholesale diet overhaul.

The results showed that at that dose, LDL cholesterol fell by 0.14 mmol/L (about 5.4 mg/dL; 95% CI, −0.19 to −0.08). Non-HDL cholesterol dropped by 0.22 mmol/L (about 8.5 mg/dL; 95% CI, −0.30 to −0.14) and apolipoprotein B by 0.08 g/L (95% CI, −0.13 to −0.03). HDL cholesterol also edged down, by 0.03 mmol/L (about 1.2 mg/dL) — a change the authors class as trivial. For beans specifically, the dose-response line kept going up to roughly 200 g per day, and each additional 100 g or so was worth another 0.25 mmol/L off LDL (95% CI, −0.48 to −0.02). The review reports those bean doses in cups rather than grams — up to 1 cup/d, with the coefficient given per 0.5 cup — so the gram figures here are converted at the paper's own equivalence of 130 g to 0.5–0.67 cup.

What the evidence does and doesn't carry. These were randomized trials, so the design does support a causal reading — pulses lowered the numbers rather than merely being eaten by people whose numbers were already lower. What limits the claim is size and certainty, not direction: the reductions are small, GRADE certainty was moderate-to-high for most outcomes but very low for apoB, and most trials ran weeks rather than years, so nothing here measures heart attacks or deaths. Chickpeas, dried peas, and lentils are also thinner on data than beans.

Why does this matter? Cardiovascular disease remains a leading cause of morbidity and mortality globally, and managing cholesterol levels is a key strategy for reducing risk. The low intake of dietary pulses in many populations highlights an opportunity for dietary improvement.

Practical takeaways. The tested amount is the useful detail: about 130 g a day is what produced these numbers — beans in a chili at lunch, lentils in a soup, chickpeas in a salad. Nothing in the review evaluates supplements, extracts, or branded products; it tested whole pulses as food.

Bottom line: About 130 g of pulses a day lowered LDL cholesterol by roughly 5 mg/dL (0.14 mmol/L) in randomized trials. That is a small effect on its own — worth stacking with other habits rather than treating as a substitute for them.
