Eighteen randomized trials, pooled together, point to one clear effect: zinc supplements make the body more sensitive to its own insulin — without touching blood sugar itself. The meta-analysis, published in Endocrinology, Diabetes & Metabolism and registered in advance with PROSPERO, combined data from 1,023 people with type 2 diabetes, gestational diabetes, or prediabetes. Most of the 18 trials gave 30 to 50 mg of elemental zinc a day for 8 to 12 weeks, though a handful ran as long as a year.
Insulin resistance improved measurably. Compared with placebo or standard care, zinc supplementation was linked to:
- HOMA-IR (a standard insulin-resistance score): -1.10 (95% CI: -2.05 to -0.15)
- Fasting insulin: -2.50 µU/mL (95% CI: -4.69 to -0.31)
- C-reactive protein (inflammation): standardized mean difference -0.91 (95% CI: -1.43 to -0.38)
- Malondialdehyde (oxidative stress): standardized mean difference -0.76 (95% CI: -1.34 to -0.18)
- Total antioxidant capacity: standardized mean difference +1.79 (95% CI: 0.68 to 2.91)
- Total cholesterol: -5.70 mg/dL (95% CI: -7.50 to -3.89)
Blood sugar itself did not move. This is the detail worth sitting with: despite the improvement in insulin resistance, the review found no significant effect on fasting blood glucose or HbA1c — the two numbers doctors actually use to track diabetes control. Zinc appears to change how efficiently the body uses the insulin it has, not how much sugar is circulating.
One number went the wrong way. LDL ("bad") cholesterol rose slightly, by 3.46 mg/dL (95% CI: 1.48 to 5.43). The review's authors call the clinical relevance of that increase uncertain, since it is a small shift, but it is a real one — and it means zinc is not a clean win across every marker.
The evidence base has real limits. Several of the 18 trials gave zinc alongside other supplements, including magnesium, vitamin D, and curcumin, which makes it hard to credit zinc alone for every effect seen. None of the included trials reported ties to zinc manufacturers, and the review's own authors declared no conflicts of interest.
Bottom line. In people with diabetes or prediabetes, 30 to 50 mg of zinc a day for 8 to 12 weeks was linked to better insulin sensitivity and lower inflammation — but not to lower blood sugar. Worth raising with a doctor if you are already managing diabetes, not a reason to skip glucose-lowering treatment.
