High-dose calcium supplements (unlike dietary calcium) have been linked in some studies to kidney stones and possibly cardiovascular risk, and calcium interferes with several medicines. Function

Calcium is both the body's main structural mineral and a universal signalling ion. Its blood level is defended so tightly that it takes part in processes all over the body:

  • Bone & teeth: about 99% of the body's calcium, stored as hydroxyapatite crystals that give the skeleton its strength — and a reservoir the body draws on when the diet falls short.
  • Muscle contraction: the direct trigger that makes skeletal, smooth and heart muscle contract; the interplay of calcium and magnesium governs contraction and relaxation.
  • Heartbeat & rhythm: controls the force and timing of each heart contraction.
  • Nerve signalling: required to release neurotransmitters and pass impulses between nerve cells.
  • Blood clotting: an essential cofactor at several steps of the clotting cascade.
  • Cell signalling: a universal intracellular "second messenger" that switches on enzymes, triggers hormone secretion and drives cell division.
  • Blood-vessel tone & blood pressure: drives the constriction and relaxation of vessel walls.
  • Enzymes & hormones: a cofactor for many enzymes and needed for secreting hormones such as insulin.
  • Cell-membrane stability and the transport of ions and fluids across membranes.

Blood calcium is kept in a narrow range by three regulators — parathyroid hormone, active vitamin D (calcitriol) and calcitonin. If intake is low, the body maintains blood levels by taking calcium from bone, which is why long-term shortfall shows up as weaker bone rather than an abnormal blood test.

Different forms — and which to choose

Choose by elemental calcium, absorption and tolerance:

Carbonate~40% elemental (most per pill) and cheap, but needs stomach acid so must be taken with food; more likely to cause bloating/constipation. Poorly suited to people on acid-reducing drugs.
Citrate~21% elemental — less per pill but absorbed with or without food and without needing stomach acid. Better for older people, low stomach acid or PPI users; gentler on the gut.
Lactate / gluconateLower elemental content; well absorbed but need larger tablets.
Hydroxyapatite / "coral"Marketed as superior; no convincing evidence of advantage over citrate/carbonate.

Practical take: carbonate with meals if you tolerate it (cheapest), citrate if you have low stomach acid, take a PPI, or get an upset stomach. Take ≤500–600 mg elemental at a time — absorption drops with bigger single doses.

Absorption in the body

How muchRoughly 25–35% of dietary calcium is absorbed, actively and dependent on vitamin D. Fractional absorption is higher when intake is low and in growth/pregnancy, and single doses ≤500–600 mg absorb far better than large ones.
HelpsAdequate vitamin D; splitting doses; carbonate taken with food.
HindersOxalates (spinach, rhubarb) and phytates (bran) in the same meal; very high intakes; older age; low vitamin D. Calcium in turn blocks iron and zinc absorption — separate them.

Deficiency symptoms

Because blood calcium is defended by pulling from bone, dietary shortfall shows up in the skeleton over years rather than as an immediate blood change.

Long-term low intakeLow bone mass → osteopenia and osteoporosis, raising fracture risk, especially after menopause.
Low blood calcium(from medical causes, not just diet) → tingling around the mouth/fingers, muscle cramps and, if severe, tetany and abnormal heart rhythm.
Who's at riskPostmenopausal women, people who avoid dairy without replacing it, vegans, malabsorption (coeliac/IBD), and low vitamin D status.

Recommended intake

Adults ≈ 950–1000 mg/day

General adult reference intakes:

  • Adults: ~950 mg/day (EFSA) · 1000 mg/day (US)
  • Women 51+ and men 71+: ~1200 mg/day (US)
  • Teens (high bone growth): ~1150–1300 mg/day

Best food sources: dairy (milk, yoghurt, cheese), fortified plant drinks, tinned fish with bones (sardines), tofu set with calcium, kale and other low-oxalate greens, almonds.12

Therapeutic use

Strong evidenceOsteoporosis prevention and treatment — always with vitamin D, and topping up diet rather than replacing it; treating medically low blood calcium.
ReasonableAdequate calcium + vitamin D in pregnancy reduces pre-eclampsia risk where intake is low; antacid use (carbonate).
Modest / mixedSmall effects on blood pressure and PMS; weight-loss claims are not supported.

Therapeutic doses

Supplement: 500–600 mg elemental, 1–2×/day
Bone healthEnough to fill the gap between diet and the ~1000–1200 mg target — often 500–600 mg elemental once or twice daily, with vitamin D. There is no benefit to exceeding the target.
How to takeSplit doses, ≤500–600 mg at a time; carbonate with food, citrate any time.

InteractionsReduces absorption of iron, zinc, thyroid medicine (levothyroxine) and some antibiotics (tetracyclines, quinolones) — separate by 2–4 hours.

Bottom line: get calcium from food first (dairy or fortified alternatives, sardines, tofu, greens). If you supplement, use carbonate with meals or citrate if acid is low, only enough to reach ~1000–1200 mg/day total, always with vitamin D, split into ≤600 mg doses — and don’t megadose.
