Digestive enzymes & stomach acid — what actually helps

Enzyme and betaine-HCl supplements are sold for bloating, gas and reflux. The honest picture: a few have genuinely strong evidence for specific problems — the broad “digestive” blends much less so.

Your body already makes its own digestive enzymes and stomach acid. Supplements aim to top these up — but the evidence is very uneven. Some are among the best-proven supplements there are; others rest on little more than marketing. Here is how to tell them apart.

How digestion works

Food is broken down by enzymes (which cut carbohydrates, proteins and fats into absorbable pieces) and by stomach acid (which sterilises food, unfolds proteins and switches on the stomach’s own protease, pepsin). The pancreas supplies most digestive enzymes — amylase (carbs), protease (protein) and lipase (fat). A healthy body makes plenty of both; supplements only make a clear difference when something specific is missing.

Enzymes that clearly work

LactaseFor lactose intolerance, taking lactase with dairy is well established to reduce gas, bloating and diarrhoea — it simply supplies the enzyme those individuals lack. A genuine, targeted fix.
Alpha-galactosidaseFor gas from beans, pulses and high-FODMAP vegetables, oral alpha-galactosidase (e.g. “Beano”) reduces gas and bloating in a dose-dependent way in randomised trials — including a placebo-controlled trial that cut moderate-to-severe bloating days.2
Pancreatic enzymes (PERT)Highly effective for exocrine pancreatic insufficiency (after pancreatitis, cystic fibrosis, pancreatic surgery). This is a diagnosed medical use, prescribed and dosed by a clinician — not something to self-treat for everyday bloating.

Broad enzyme blends

The multi-enzyme “digestive” capsules sold for general bloating are a different story. A few small, often industry-funded trials report reduced bloating after a heavy meal,3 but overall the evidence is limited and low-quality, and there is no good case for a healthy person with a normal pancreas to take them routinely. If a specific food is the trigger, a targeted enzyme (lactase, alpha-galactosidase) is the smarter, better-proven choice.

Stomach acid & betaine HCl

Betaine hydrochloride (HCl) is sold to raise stomach acid in people believed to have low acid (hypochlorhydria) — a popular idea in wellness circles, often blamed for reflux and poor digestion. The reality is more modest:

What it doesIn people with drug-lowered stomach acid, 1500 mg betaine HCl briefly dropped gastric pH from ~5.2 to ~0.6 — but the effect was short-lived (often gone within ~75 minutes).1
What it doesn’tThere is no validated dosing from good trials, no outcome evidence that it fixes symptoms or improves nutrient absorption, and much of the marketing rests on testimonials.1
A cautionGenuine low stomach acid is uncommon, and reflux is far more often not a low-acid problem — taking acid can make an ulcer or gastritis worse. This one deserves a clinician’s input, not self-diagnosis.

Safety & cautions

Betaine HClAvoid with a history of ulcers, gastritis or reflux, or while taking NSAIDs or acid-suppressing drugs (PPIs); it can cause a burning sensation. Use only with professional guidance.
EnzymesGenerally well tolerated; mild GI upset is possible. Those with allergies should check the source (e.g. pork-derived pancreatin, fruit-derived bromelain/papain).
Don’t mask a problemPersistent bloating, reflux, weight loss or pain deserve a diagnosis — don’t self-treat ongoing symptoms with enzymes or acid indefinitely.
Bottom line: targeted enzymes (lactase for dairy, alpha-galactosidase for beans) are genuinely useful; pancreatic enzymes are powerful but medical; broad “digestive” blends and betaine HCl are, for most healthy people, optional and weakly supported.
