What is actually proven
Give this side its full due, because it is real. In randomized, placebo-controlled human trials, nicotinamide riboside reliably increases blood NAD+ in a dose-dependent way and is well tolerated.1 Longer trials confirm the rise is sustained over weeks, with hints — not proof — of downstream effects like a small drop in blood pressure in one subgroup.2 NMN shows the same core result: it raises NAD+ metabolites, and one trial in prediabetic women reported improved muscle insulin sensitivity, a biomarker outcome.4 So if your question is narrowly “does this pill raise the molecule it claims to?” the answer is yes. That already puts NAD+ boosters ahead of most of this shelf.
What is not
Here is the part the ads skip. Raising a biomarker is not the same as improving your health, and NAD+ is a textbook case of the difference. No human trial has shown that boosting NAD+ makes people age more slowly, think more clearly, recover faster or live longer. The studies are small (often dozens of people), short (weeks to a few months), and designed to measure blood chemistry, not real outcomes. The dramatic results — restored fur, better endurance, longer life — are almost all in mice. Betting that mouse lifespan data transfer to humans is exactly the bet resveratrol lost. Until a proper trial measures something that matters and finds it, the correct verdict is “plausible, unproven.”
The NMN regulatory mess
NMN has a specifically awkward status in the United States. In 2022 the FDA concluded that because NMN had been authorized for investigation as a new drug, it is excluded from the legal definition of a dietary supplement — effectively telling sellers it should not be marketed as one.3 Enforcement has been patchy and products remain widely available, but the label “dietary supplement” on an NMN bottle is on shaky legal ground, and quality/purity vary a lot between brands. NR does not share this problem — it is an established, notified supplement ingredient — which is one practical reason people who want to try a NAD+ booster often choose NR.
Dose, safety and the verdict
Trial doses run roughly 250–1,000 mg/day for NR and a similar range for NMN. Both look safe and well tolerated over the months studied, with no serious signals; what we lack is long-term data. If you want to experiment with a marker you can even measure, that is a defensible personal choice — pick a reputable brand, prefer NR on the regulatory grounds above, and be clear-eyed that you are buying a raised number, not a proven benefit. What you should not do is treat it as established anti-aging medicine, spend heavily on it, or let it displace the things with real outcome evidence: exercise, protein, sleep.
