Black seed — also called black cumin, though it is unrelated to the spice cumin — is the seed of Nigella sativa, used in traditional medicine across the Middle East, North Africa and South Asia for centuries as a remedy for everything from cough to digestive complaints. Interest in its effect on weight and cholesterol is more recent, but there is now a real body of human clinical trials behind it — not just tradition. Here is what those trials found, and the proposed reason why.
What the trials looked at
A 2022 literature review pooled 14 human clinical trials and 5 animal studies testing Nigella sativa specifically against body weight, waist circumference and related metabolic markers.1 The human trials ran on a mix of populations — people with type 2 diabetes, obese and overweight adults, people with non-alcoholic fatty liver, and people with elevated cholesterol — using either whole ground seed powder or cold-pressed seed oil, taken daily in capsules for 6 to 12 weeks or longer.
The most consistent, best-controlled results came from double-blind, placebo-controlled randomized trials using black seed oil rather than powder, generally in doses around 1–3 grams a day.
Effect on weight and BMI
Ten of the fourteen clinical trials in the review found a significant reduction in body weight, BMI, or waist circumference with Nigella sativa versus placebo. The effect was most consistent with oil rather than powder, and required sustained daily use — most positive trials ran 6 to 12 weeks or longer.
One well-controlled example: a crossover trial in 39 women with a BMI of 27–35 gave 1 gram of black seed oil twice daily for 8 weeks, then crossed the groups over. It found significant reductions in body weight, waist circumference, body fat mass and visceral fat area — and, notably, participants reported feeling less hungry on a standardized appetite scale, without any measured change in how much they actually ate.2 Two separate randomized trials pairing 3 g/day of black seed oil with a low-calorie diet in obese women also found greater weight and waist-circumference reductions than the low-calorie diet plus placebo alone.56
Not every trial agreed — four of the fourteen found no significant difference in body weight. The studies are mostly small (40 to 140 participants), short to medium in duration, and not all were blinded, so this is promising evidence rather than a settled conclusion.
Effect on blood lipids
Several of the same trials, plus others run specifically on cholesterol and metabolic risk, found significant reductions in total cholesterol, LDL (“bad” cholesterol) and triglycerides with Nigella sativa oil.
In a randomized, double-blind, placebo-controlled trial of 43 adults with type 2 diabetes, 1 gram of black seed oil daily for 8 weeks significantly lowered HbA1c, total cholesterol, LDL and triglycerides compared with placebo.3 A separate randomized trial in 117 obese prediabetic adults compared Nigella sativa oil against both lifestyle modification and metformin, and found the Nigella sativa group had significant reductions in total cholesterol, triglycerides, LDL, fasting glucose and insulin resistance, alongside the weight loss.4
The effect on HDL (“good” cholesterol) is less consistent across trials than the effect on LDL and triglycerides — some show no real change, a few show an increase. If cholesterol is your main reason for interest rather than weight, LDL and triglycerides are where the evidence is strongest.
The proposed mechanism
The main active compound in black seed oil is thymoquinone, alongside the oil's own fatty acid profile. Researchers studying these trials have proposed several mechanisms, likely working together rather than any single one acting alone:1
- Appetite suppression — shown directly in the crossover appetite trial above, independent of any deliberate dieting.
- Reduced intestinal glucose absorption, which would blunt the insulin spikes that promote fat storage.
- Anti-inflammatory and antioxidant effects — several trials measured improved insulin sensitivity and lower markers of inflammation alongside the metabolic improvements, a pathway that would reduce fat storage indirectly rather than “burn” it directly.
- Effects on fat-tissue signaling — early research has looked at changes in proteins involved in how fat cells form and store energy.
One reassuring detail from the review: the weight and lipid changes are not a side effect of feeling unwell. In trials involving people with fatty liver, Nigella sativa lowered elevated liver enzymes rather than raising them — consistent with a genuine metabolic improvement, not appetite loss from nausea or discomfort.
What the evidence doesn't show
This is encouraging early-to-moderate clinical evidence, not proof of a dramatic effect. The trials are small by pharmaceutical standards, mostly conducted in Iran and Egypt, mostly 6–12 weeks long, and several used open-label designs without blinding. Nigella sativa is not a substitute for established weight-management or cholesterol-lowering approaches — diet quality, activity, and where relevant, medication remain the foundation. Anyone on blood-thinning medication, diabetes medication, or with a scheduled surgery should check with a doctor before starting Nigella sativa oil, since it can interact with both blood sugar and blood clotting.
